AUTHORIZE
The customer identified above, by checking this box, hereby authorizes dockbox and its financial institution, to initiate and/or transmit one (ACH) debit entry to the customer’s account identified herein below. The parties agree and understand that this ACH authorization is for a single debit entry only authorized for a single reversal. The customer acknowledges that the purpose of this ACH release is for the payment to dockbox for the one-time provision of goods or services from dockbox store to the customer and that this constitutes a valid business relationship between the two parties. The Customer agrees to hold dockbox and/or Partner’s financial institution harmless from any and all claims associated with compliance with the single use ACH authority. Additional data cost will apply when using ATM monitoring service. customer agrees to provide a voided check to verify customer’s account and routing numbers. Any and all services and equipment purchased through dockbox are for the sole use on ATMs only. Any units used with any other purpose or on any equipment other than an ATM machine will be shut down immediately and customer will incur a 125.00 usage breach fee. Additional fees may apply.

Authorize Debits Monthly The customer identified above, by checking this box, hereby authorizes dockbox and its financial institution, to initiate and/or transmit Monthly (ACH) debit entries to the customer’s account identified herein below. The parties agree and understand that this ACH authorization is for Monthly debit entries. Customer acknowledges that the purpose of this ACH authorization is for the payment to dockbox ATM Store for monthly provisions of goods or services from dockbox to the customer and that this constitutes a valid business relationship between the two parties. The customer agrees to hold dockbox and/or customer’s financial institution harmless from any and all claims associated with compliance with the Monthly use ACH authority. The customer agrees to provide a voided check to properly validate customer’s account and routing numbers. Any and all services and equipment purchased through dockbox are for the sole use on ATMs only. Any units used with any other purpose or on any equipment other than an ATM machine will be shut down immediately and customer will incur a 125.00 usage breach fee. Additional fees may apply. (not applicable when purchase only option)Agree to Return Policy DOCKBOX STORE RETURN POLICY: You must return items in new or unused condition with all original materials included with the shipment. All returns must be associated with an RMA number provided by dockbox. We must receive your returned items at our warehouse within 10 days of delivery. We inspect all returned items and return only a partial refund for opened or used products. Damaged units will not be refunded and customer will be charged for unit replacement. No shipping or credit card charges will be refunded. You can expect your refund within 1 to 2 billing cycles. All dockbox refunds are issued to the original method of payment. Term agreement commences on date shipped, in the event that this agreement is terminated prior to the agreement end date an early termination fee of $350.00 will be charged plus all network fees on the remaining months of agreement. |
